Formalities

As in any hospital establishment, you must complete certain administrative formalities when you are admitted to the Clinique Chantecler.

Pre-admission

On the day of the pre-anaesthetic consultation, remember to go to the pre-admissions office beforehand so that you can be registered administratively. You will be asked for the documents you need for your future admission, so as to simplify the procedure and the waiting time when you arrive at the clinic on the day of your hospitalisation or operation.

Opening hours

  • Mondays, Tuesdays and Thursdays from 10:30 to 12:30 and from 13:30 to 18:00.
  • Wednesdays and Fridays from 9.00am to 12.30pm and from 1.30pm to 3.30pm.

The followingdocuments are required:

  • Your National Identity Card or your Passport or your Livret de Famille,
  • Your updated "Carte Vitale d'Assuré Social",
  • Your Mutuelle or Insurance Card,
  • The Work Accident Vouchers, if applicable,
  • For the hospitalisation of a child aged over 16: a School Certificate,
  • For the unemployed: the last 3 notices of payment from ASSEDIC, if your rights are closed on your Carte Vitale,
  • For CMU beneficiaries: Attestation de prise en charge or Carte Vitale,
  • For foreign nationals from the E.E.C.: duplicates of forms E 111, E 112 AND your European Card,
  • Your 100% certificate if you are a beneficiary,

Admission

The admissions office isopen at the followingtimes:

  • Weekdays from 07:30 to 17:30

You must present the documents you were missing during pre-admission, the completed pre-admission file and the tests carried out prior to admission.

If you do not present all the necessary documents, your admission may be postponed. A deposit cheque will be required in the event of hospitalisation, as well as for television if you wish to have it.

Compulsory contributions linked to your stay

In accordance with current regulations, you will be asked to pay certain fees. These are not reimbursed by the Assurance Maladie but may be covered by your mutuelle or complementary health insurance. Ask your health insurance company for details. There are exemptions, particularly for people receiving complementary health insurance or state medical aid. Fixed hospital charge: €20 per day (including the day of discharge)

The fixed hospital charge represents your financial contribution to the accommodation and maintenance costs incurred by your hospitalisation. It is payable for each day of hospitalisation, including the day of discharge. It does not apply to outpatient stays. The amount of the fixed hospital charge is set by ministerial decree.

Fixed contribution: €24 per stay

Preserving the social security system is everyone's business and, in principle, you contribute to it for each of your healthcare expenses. The part of the cost that remains payable by you is known as the "fixed contribution" or "co-payment".

The flat-rate contribution of €24 applies to procedures costing €120 or more. You may be asked to pay a co-payment for any procedure under €120.

You may not be billed for any costs other than those corresponding to the care provided or, where applicable, to any special requirements you may have requested. The amount of these special requirements, the list of which is strictly defined by the regulations and includes in particular access to a private room, must be communicated to you before the treatment is carried out.

From the A50: Take exit 5 "aéroport Marignane/Aix-en-Provence/Fos-sur-Mer/La Penne-sur-Huveaune/Saint Menet/La Valentine", continue towards La Valentine, then take the avenue "des peintres roux", the clinic is on your left.

From the A7: Take the "Belle de Mai/Cinq-Avenue" exit and join the Viaduc de Plombières/D4C. Turn left towards Malpassé/La Rose, then left towards Aubagne/Toulon/La Valentine/St Barnabé. Continue along Avenue des Olives towards La Valentine, with the clinic on your right.
